The Montana End-of-Life Registry service provides registered health care providers and consumers with access to stored advance health care directives. There is no cost to use this service.

For information, forms and other resources about filing and changing advance directives, see the Department of Justice Advance Health Care Directives page. Advance directives may not be submitted electronically with this service.

General Tips

When using the Registry service to search for an advance directive, keep the following in mind:

Adobe Reader Required to View Advance Directives

Advance directives are saved in the portable document format (PDF), and Adobe Reader is required to view PDF files. For help with viewing or printing PDF files, go to the Department of Justice Help page.

Using the Registry Service – For Consumers

People who have filed an advance directive with the Montana End-of-Life Registry can access and view the directive.

  1. In the Registrants box, click Start.
  2. Enter:
  1. Click View.
  2. If an advance directive is on file, a PDF copy of it will appear on the screen.

Health care providers who are registered with the Montana End-of-Life Registry can search for and view advance directives.

If you know the patient's name and access code from his or her wallet card:

  1. In the Health Care Providers box, click Start.
  2. Enter the patient’s name precisely as it appears on his or her wallet card.
  3. Enter the patient’s access code.
  4. Click View.
  5. If an advance directive is on file, a PDF copy of it will appear on the screen.

If you need to search the database of registered advance directives:

  1. In the Health Care Providers box, click Start.
  2. Enter your health care provider user name/ID and password.
  3. Click Login.
  4. Enter search criteria.
  1. Click Search.
  2. If an advance directive is on file, a PDF copy of it will appear on the screen. If more than one advance directive matches the search criteria, a list will appear on the screen. Click on the appropriate name to view a PDF copy of the directive.
